Safe Drug Disposal Stewardship Ordinance - Notice of Plan Operator Change Form

Under San Francisco's Safe Drug Disposal Stewardship Ordinance, Producers (i.e., pharmaceutical manufacturers that sell Covered Drugs in San Francisco) are required to notify the Department of any changes to their Plan Operator or participation in a new Stewardship Plan. To notify SFE, Producers must complete this form and submit it by email to [email protected].
